Client Referral Source Tracker (FREE)

The Client Referral Source Tracker Spreadsheet provides you with a framework track where your first clients / patients are coming from.

The spreadsheet allows you to record the following information:

• Patient Name, Initials or Alias - a way to track who they are (always remember to keep patient names in a way that is compliant to the regulations in the country you are practicing in)
• Referral Source - who referred this client to you (e.g. clinic, word-of-mouth, a specific therapist directory or matching platform?)
• Date of referral - when was this client referred to you (this can show you trends over time. For example, at the start you may have most of your referrals come from a matching platform, but over word-of-mouth becomes your main source of referrals, which is very normal)
• Therapy / Treatment -this may show you that different referral sources send you clients with different therapeutic needs)
• Rate - always good to know
• Notes (overall sentiment, reason for referral, etc.) - capture anything else relevant here

The spreadsheet is provided in Microsoft Excel format (xlsx). It is designed to be a starting point so feel free to add additional columns to capture additional information that's missing.
